A completed Intake Form (click the link below) is required for all
presentations to the Multi-Disciplinary Team or the Family Stability Team and
will be the only form accepted in order for us to track data required by the state.
Please complete the form electronically and complete a
separate Intake Form for each child presented.
A completed referral packet for residential treatment or presentations for ideas
must be received by Tim Kehres (Tim.Kehres@jfs.ohio.gov) no later than
noon two business days prior to the scheduled presentation. If any
information is missing, Tim will have time to return the form to the presenter
and missing information can be included so the presentation can go forward.
